 MEDICAL, WELLNESS CREDIT            10/11/2018

Are you eligible for the 2019 Wellness Credit?
In 2019, you are eligible to receive an annual credit of $100, prorated on a per pay period basis toward your medical contribution, if you have obtained a preventive, routine, “well-adult” physical exam with applicable biometric measurements in 2018. 
To receive the $100 Wellness Credit in 2019, during the 2019 annual benefits renewal window, you must self-identify that you have obtained (or will obtain by 12/31/2018) your annual physical exam. The 2019 Annual Benefits Enrollment period is October 31 – November 16, 2018.
A preventive well-adult exam is performed by your primary care physician (PCP). A Well Woman Exam is not counted as a “Routine Annual Physical” for women. 
Applicable biometric measurements vary depending on age, gender, etc. Some examples of biometric measurements include height, weight, body mass index, blood pressure, etc. 
It is preventive care service and covered at 100% one-time annually by the Medical Plans. It is billed by your PCP to Aetna/Anthem with a preventive, wellness examination procedure code. Make sure your doctor submits the claim as “Routine Annual Physical”. 
There are no forms to fill out or send in. To receive the credit, simply obtain your annual physical exam and indicate you’ve done so during the annual benefits renewal period.

ï‚· If everyone in the United States received recommended clinical preventive care, over 100,000 lives would be saved each year.
ï‚· Preventive care includes health services like screenings, check-ups, and patient counseling that are used to prevent illnesses, disease, and other health problems, or to detect illness at an early stage when treatment is likely to work best. Getting recommended preventive services and making healthy lifestyle choices are key steps to good health and well-being.
